It seems like there is a vscode/webview bug that sometimes causes `postMessage` to hang indefinitely

Ensures updateRichState uses the correct current context

- Allows for multiple providers at once
 - Avoids the need to set "github-enterprise.uri" setting

- No functional changes, only file splitting, renaming, etc

Renames Authentication to be clearer about remote provider connections
